Responsibilities of nurses at the beginning of dialysis :
- Checking the last tests of patients
- Making decisions regarding the correction of electrolyte disturbances according to the department's protocols and according to the patient's tests
- Checking the patient continuously by head nurse and timely response to the patient's request for help
- Training patients like self-care based on the individual's needs, such as nutrition, fistula care methods, and the use of medicine or the patient's possible problems.
- Paying attention to the socio-economic problems of patients and talking to them and helping them
- Controlling vital signs, the injection site and the condition of the device continuously in the case of stable patients
- Controlling vital signs, the injection site and device status continuously for unstable patients and emergency cases
- Injecting Heparin continuously or frequently
- Controlling the patient's blood flow in terms of the presence of clots
- Controlling the patient's vessels like controlling the blood pressure increase in them
- Treating with patient’s request properly
- Visiting patients with doctors
- Paying attention to the patient's condition, emergency treatment measures in necessary cases and informing the doctor of special cases
- Handing over patients to other nurses when the shift is over
- Paying attention to the time of dialysis
Responsibilities of nurses when the dialysis ends:
- Checking the vital signs before the end of dialysis to find if serum is needed
- Washing hands before disconnecting patients to the device
- Returning the patient's blood completely and in a sterile manner
- Separating the needles and collecting them in the stiffness of the box
- Separating the sets and putting them in the garbage bag so as to cause the least pollution.
- Keeping the injection site in case of unwell or lethargic patients
- Controlling the injection site bleeding and putting a sterile dressing on the needle injection site
- Rechecking the patient's vital signs and weight
- Injecting Erythropoietin correctly and controlling blood pressure
- Ensuring proper weight loss of the patient
- Ensuring that the patient is stabilized to leave the ward
- Writing a dialysis sheet with a complete report in the patient's file (entering the patient's consumable items in the HIS system
- Supervising the disinfection of the device and the ward’s scrub according to the ward’s protocol
- Turning off the device and RO water purification system
There are 4 beds equipped with 4 Dialysis devices. Patients undergo dialysis in morning/ evening shifts, and night shift if needed. Technical official of the ward is an internist.
